VHSL PHYSICAL
Physicals must be completed and dated by a physician after May 1 of the current school year. You can find the updated form (and more information) on the Woodson Athletics page.
Athletes can submit completed forms via ONE of the following options:
1. Email a copy to our Athletic Trainer, Chris Hogge, at [email protected]
2. Mail it to: Woodson High School Attn: Activities Office 9525 Main Street Fairfax, VA 22031
3. Drop it off in the Woodson Main Office between 8:00a.m. and 3:00p.m. Monday – Friday
FCPS TRANSPORTATION FORMS
FCPS Field Trip Forms are required for all transportation in relation to Woodson Rowing events. These forms allow students and parents to drive athletes to all events related to rowing. Please fill out one driver form for each possible driver (student, parents, step-parents, etc.)
- Drivers License and Insurance Form (for ALL drivers, both athlete and parent/guardian)
- Student permission to ride with other parents
- Student permission to ride with other students
Completed forms shall be provided to the Woodson Rowing Membership Chair. Hard copies with signatures are required. Emailed/electronic copies of the forms will not be accepted.
WOODSON ROWING TRAVEL REGATTA FORMS
The following forms are only required once an athlete is selected to attend a travel regatta. Completed forms shall be provided to the VP Activities. Hard copies with signatures are required. Emailed/electronic copies of the forms will not be accepted.
