PHYSICALS
Physicals must be completed and dated by a physician after May 1, 2023 in order to be used for the 2023-2024 school year. You can find the updated form each year on the Woodson Athletics page.
Athletes can submit completed forms via ONE of the following options:
1. Email a copy to our Athletic Trainer, Chris Hogge, at [email protected]
2. Mail it to: Woodson High School Attn: Activities Office 9525 Main Street Fairfax, VA 22031
3. Drop it off in the Woodson Main Office between 8:00a.m. and 3:00p.m. Monday – Friday
TRANSPORTATION FORMS
FCPS Field Trip Forms are required for all transportation in relation to Woodson Rowing events. These forms allow students and parents to drive athletes to all events related to rowing. Please fill out one driver form for each possible driver (student, parents, step-parents, etc.)
- Driver form (for both athletes with driver’s licenses and for parents)
- Student permission to ride with other parents
- Student permission to ride with other students
Completed forms shall be provided to the Woodson Rowing Membership Chair. Hard copies with signatures are required. Emailed/electronic copies of the forms will not be accepted.
TRAVEL REGATTA FORMS
The following forms are only required once an athlete is registered with a team and IF the athlete is selected to attend a travel regatta. Completed forms shall be provided to the VP Rowing Activities. Hard copies with signatures are required. Emailed/electronic copies of the forms will not be accepted.

- Luggage Check-In Form (submitted in person at luggage drop-off)
- Student Medication Form (if it applies, submitted in person at luggage drop-off)
