FORMS FOR THE FOLLOWING:
REGULAR SEASON CARPOOLING , LOCAL REGATTAS, & ST. ANDREWS/TRI-MEET
FCPS Field Trip Forms are required for all transportation in relation to crew events. These forms allow students and parents to drive athletes to all events related to crew. Please fill out one driver form for each possible driver (student, parents, step-parents, etc.)
- Driver form (for both athletes with driver’s licenses and for parents)
- Student permission to ride with other parents
- Student permission to ride with other students
Completed forms shall be provided to the Woodson Rowing Membership Chair. Hard copies with signatures are required. Emailed/electronic copies of the forms will not be accepted.

STOTESBURY AND SRAA NATIONALS
The following forms must be completed, printed, signed and turned in to the Membership Chair prior to each event. Hard copies with signatures are required. Emailed/electronic copies of the forms will not be accepted.

- Parental Authorization Form
- Luggage Check-In Form (submitted in person at luggage drop-off)
- Student Medication Form (if it applies, submitted in person at luggage drop-off)
